Depression, OCD and fertility

Hi,

I suffer from depression and OCD since my teenage years.

I took fluoxetine (Prozac) since 2007 and this was a life saver. I was so successful that I dared to stop/taper it end of 2021. In June 2022 it took 4 weeks until depression got me and I needed to return. This time I tried sertraline (Zoloft). I took it for a year and after having headaches and because of trying to get a better fertility so my wife and I could get a baby I stopped again with the help of my psychiatrist in August 2023. Now in the dark and cold time in mid Europe I am suffering from the return of my depression and obsessive thoughts and fears. I can’t leave bed in the morning and I have diffuse fears about many things. I even wished that everything to be stopped and I be gone for good. No state to become a father just to make the sperm better.

I tried but I failed to be without the meds.

My Andrologist told me to just return to SSRI.

I did a research myself: Duloxetine had a human trial and should be safe for fertility and also help with CPPS: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8487983/

So at least I decided not to continue in my miserable state. But I need to choose my medication wisely. I will have an appointment with my psychiatrist next week.

Anyone here who has some experience with my problems?
Age: 41 Onset Age: 14, 22 got really bad
Symptoms: pain and discomfort perineum, a little bit while sitting in the right half of the scrotum. Sometimes dual stream while urinating. While ejaculating, not all shoots outside, some stays and flows outside with time (Without medication very painful).
Diagnosis: have seen several urologists in my life, have done any possible and pain creating diagnosis could not help me ever. Life was hopeful after googling the Stanford/Wise-Anderson Protocol in early 2008, then every ache started making sense./ Hemorrhoids: they stress my prostate when I have a Hemorrhoid flare!
Helped By: Physical therapy, keeping myself warm, hatha yoga, stretching, walking, relaxation, distraction, Tadalafil 5 mg
Worsened By: Constipation, Fear or OCD "flare up", sitting and not doing any kind of "motion", too much tension in media or life, caffeine and acidy food
Prior Treatments: Antibiotics, Tamsulosin, Lyrica, Gabapentin, Tylenol

PS: now I read that Duloxetine can cause testicle pain?!

And most SSRI tests that hurt sperm had been with animals. I found only one human trial with paroxetine

There is also only one study with duloxetine which was only for 6 weeks and no control if the test group really took the medication or not.

I really get sick not knowing which medication I should get with a good feeling.

If you are not scientifically or medically trained, you are well advised to KEEP AWAY from doing your own research. You are a prime example of why. You have found a rare side effect of a drug, which occurs in less than 1% of patients, and started catastrophizing about it.

Regarding SSRIs and sperm, a recent review study found that SSRIs do affect sperm negatively. So if making babies is your big priority, I would not use them. Duloxetine, a SNRI not a SSRI, is okay.

You sound as if you have S.A.D. (seasonal affective disorder). Get Light Therapy and try a non-SSRI antidepressant.

User-CPPS, I was wondering how everything has been?

I am taking Fluoxetine 20 mg and I have no depression and ocd is manageable. No effect on my pelvic pain, I manage it, I did not conquer it…yet
